Qualification procedure and booking a vasectomy date

1. 1. book the date of the procedure by phone (+48 607 776 777).
   2. fill in the health questionnaire (as soon as possible after the date has been booked).
   3. the patient will receive a confirmation at the e-mail address provided, together with the wording of the informed request and consent to the procedure.
   4. the doctor performing the procedure will call the booked patient (usually 1-3 days before the procedure, or earlier if any additional tests or explanations are required).
      The phone conversation and the completed health questionnaire are part of the medical history and an integral part of the qualification process.
